With a vision to offer a complete solution for patients who select to have individualized care delivered to them in the comfort and security of their own home.  We provide comprehensive services that promote independence, delivered by our team of trusted and highly credentialed caregivers that combine innovative approaches to care with compassion and concern.

Meridian at Home Hospice provides hospice services to patients in Monmouth and Ocean counties.  Hospice is a philosophy of care that focuses on the comfort of the patient and not curative treatment.  Hospice is care for individuals who are terminally ill, with six months or less to live.  Volunteers with hospice can expect to gain experience working with patients in a companionship role and / or help support clerical functions in our Ocean County or Monmouth County offices. Training is provided.
